When are you interested in getting a new puppy? What is your timeline?

Have you personally owned a pet before (meaning he/she was completely YOUR responsibility)? If not, are you ready for the responsibility of owning your own pet and why do you feel you are ready?

If you answered “Yes” to the first part of the above question, how many pets do you currently have in your home? What kinds? Breeds? Sizes? Ages? Are they spayed/neutered? If not, have they ever been used for breeding?

Do you now or do you ever plan to breed in the future?

If you are approved and you purchase a puppy from me, would you be willing to sign a spay/neuter contract?

Are you financially able to take on the vet bills for a new puppy right now (heartworm prevention, check-ups, teeth cleanings, spay/neuter, shots, flea/tick prevention, etc.)?

How much money do you expect to spend per month on a puppy’s food, vet bills, supplies, etc.?

Have you ever surrendered an animal before? If so, please give details.

If you are approved and purchase a puppy from me, and you should EVER have to give your puppy/dog up, no matter the age or circumstance, are you willing to return him/her to me? (This is a REQUIREMENT for getting a pup from me, I must retain first right of refusal and be offered the puppy back if the owner cannot keep him/her at any time. Immediate family members would most likely be able to keep the puppy in the event that the original owner could not, but I would need their contact information and they would need to sign a new contract with me. If you do not feel comfortable with this requirement I apologize, but I must protect my puppies and I must know where they are for their lifetimes.)

Would you consent to a home visit/check either before you are approved for a new puppy from Madaloo Yorkies/Biewers or anytime after you take home your puppy/dog if you are notified in advance (at least 24 hours) that the visit will be done? (Note: This is not usually a requirement to be approved for one of my pups, but I do like to have the option if I feel it is necessary in a certain situation.)
